Choose a Select Plan primary care provider to deliver or arrange for your care, including referrals to specialists.
What to know before you choose this plan:
The Select Plan network is smaller than the Choice Plan network.
There is no Tier 3 coverage with the Select Plan. Except in an emergency, you can receive care only from Select Plan (Tier 1 and Tier 2) providers and hospitals.
If there are certain providers and hospitals you want to visit, please make sure they participate in the Select Plan network and that the providers have admitting privileges at Select Plan hospitals.
