Fertility services
Many Harvard Pilgrim plans cover certain fertility services for eligible members. Covered services typically include:
- Specialist consultation (PCP referral required for HMO members)
- Diagnostic services medically necessary to assess fertility, such as:
- Lab work
- Hysterosalpingogram
- Laparoscopy
- Ultrasound
- Sperm testing
- Fertility support services and procedures, including:
- Intrauterine insemination (IUI)
- In vitro fertilization (IVF)
- Frozen embryo transfer (FET)
- Gamete/zygote intra-fallopian transfer (GIFT/ZIFT)
- Microsurgical sperm aspiration (MESA)
- Testicular sperm extraction (TESE)
